To complete your eAdmission, please ensure you have access to the following:

• A mobile phone and email address
• Your healthcare cards, including:
  • Medicare card
  • Private health insurance details
  • Concession card (if applicable)
  • Other insurance details (eg WorkCover or TAC, if applicable)
  • Ambulance membership card (if applicable)
• Your personal details and next of kin information
• Contact details for your usual General Practitioner (GP)
• A list of your current medications, including prescribed and over-the-counter or supplements (with dose, strength, and reason for use)
• Your medical and surgical history
• Any documents provided by your admitting specialist, such as your booking letter and consent form

1. On the eAdmissions login page, go to the “New user?” section Click the “Create User” button

   

2. Enter your details and follow the prompts to complete registration

Please note:

• You only need to create an account once
• If you have previously registered, do not create a new account — instead, log in using your existing email and password

  

• If you can’t remember your login details, use the “Forgotten Password” option

If you are having trouble creating an account, please check the following:

• Make sure all required fields are completed correctly
• Ensure your email address has not already been registered. If you may have already registered previously, please try logging in or use the “Forgot Password” option instead of creating a new account.
• Check that your password meets the security requirements. Your password must be at least 8 characters and include a mix of uppercase and lowercase letters and a number. Avoid using your name, email, or common words like “password.”
• Confirm you have entered the correct mobile number, as this may be used for verification. If you did not receive a verification code, check your spam/junk folder or ensure your mobile number is entered correctly.
• Make sure you have a stable internet connection

If you do not have your own mobile phone or email address, you may use the contact details of a trusted family member or friend to assist you with the process.

Currently, forms are available in English. If you need assistance, a family friend or relative may be able to assist you.

No, you do not need to create a separate account.

You can use your existing eAdmissions account to complete forms for your child, spouse, or another family member.

When you log in:

1. Select “Add New Patient” (or similar option)
2. Enter the details of the person you are completing the admission for
3. Start a new admission and complete the required forms for them

You can manage multiple family members under the same account.

Your password must be at least 8 characters and include a mix of uppercase and lowercase letters and a number. Avoid using your name, email, or common words like “password.”

Try searching for your doctor using their last name and select them from the list.

To refine your search, you can enter their name in the format:

Last name, First name (eg Jones, Brian)

Please allow a few seconds for the results to load.

If you cannot find your GP in the list, select “I cannot find my GP”.

This will allow you to manually enter your GP’s details, including their address.

 

 

If you are unsure of your GP’s address, you can:

Contact your GP clinic directly, or Look it up online

Please note: entering your GP’s address is a required field to complete your eAdmission.

Helpful tip:

You can usually find your GP’s details on your clinic’s website or previous medical documents.

If the GP details shown are not correct, select “I cannot find my GP”.

This will allow you to enter your GP’s correct details manually, including their address.

Make sure the details you enter are up to date so your information can be shared with your GP if needed.

Once you submit your eAdmission, your information is sent directly to the Patient Services team and added to your patient record.

In the lead up to your admission, you can expect:

An email confirmation to let you know your eAdmission has been received

A phone call from a nurse (usually the day before your admission) to confirm your admission time and provide any preparation instructions, such as fasting

You can upload any required documents directly through the eAdmissions portal. During your eAdmission, you will have the option to upload documents on the second last page of the form.

You can upload files in the following formats:

• PDF
• JPEG / JPG

• Make sure your documents are clear and easy to read
• Have your files ready before starting to make the process quicker

If you’re unable to upload documents, you can bring printed copies with you on the day for our staff to scan into your electronic patient record.  

Yes, you will need to complete an eAdmission each time you come to the hospital. When you log in to your account, you can start a “New Admission”. Your previous information will be automatically filled in, so you only need to review and update any details that have changed.

Please ensure all required fields are completed. Fields marked with an asterisk (*) must be filled in before proceeding. Any missing responses will have red text next to it prompting for an answer. 

 

If you are being admitted to Sydney Adventist Hospital on the day of your procedure, you will be contacted by nursing staff in the later afternoon or evening the working day prior to confirm your admission time and pre-op requirements including fasting.

 

If you are being admitted the day before your procedure please arrive at the Sydney Adventist Hospital and attend the Access Centre of Level 4 of the Clifford Tower between 4.00pm - 5.00pm unless your doctor has specified otherwise.

If you are being admitted to San Day Surgery Hornsby, staff will contact you in the afternoon of the working day prior to confirm your admission time and pre-op instructions including fasting.

If there is sufficient time after you complete your online admission, you should receive an estimate of the hospital costs from our Patient Services team before your admission date.
